Description
The aim of the project is to develop innovative social services to persons with diabetes - children, young people and adults, including persons with disabilities, through activation, information and education of persons at risk of exclusion to improve the quality of life of these groups and to promote social inclusion at the Latvian Diabetes Federation.
The target group is children and young people with disabilities, adults including pre-retirement and retirement age and persons with disabilities.
In the framework of the project there will be social rehabilitation program implemented including new non-existing e-service for persons with diabetes - "Digital Diabetes Diary", psychologist service at home, social and work skills development workshops for children and young people also with disabilities as well as support and education groups for families with children with disability due to diabetes.
The project activities are planned to be implemented at the national level - in all Latvian Planning Regions, including Latgale planning Region.
Project results: The project aimed to develop innovative social services for people with diabetes - children, young people and adult persons, including persons with disabilities, Latvian Diabetes Federation in the implementation of exclusion risk groups activation, information and education to improve the quality of life of these groups and prevent social exclusion.
The target group was the social exclusion and poverty and at risk people, including 20 children, 32 young people, 15 families with children, 129 persons with disabilities, 254 adults and 854 pre-retirement and retirement age persons. As well as NGOs - 30 members, 5 staff members and 10 volunteers.
During the project we achieved the following results:
• Implemented a social rehabilitation program, in which 513 social workers provided counselling, psychological counselling and 664 583 advice from healthcare professionals. As well as organized 79 educational support measures and set up a support group for families caring for children with disabilities.
• Implemented the LDF members, staff and volunteer training.
• Implemented information materials and information technology solutions for how to innovate the service - Diabetic digital diary, as well as improved organization's website.
• Implemented promote cooperation and participation in national and international cooperation between NGOs and networks activities. Organized two seminars for the exchange of experiences with Norwegian partners - Norwegian Diabetes Association. Participation in the Diabetes Research Association Congress and implemented an exchange visit to the Slovenian Diabetes association.
The project directly contributes to sustainable social security system parameters and tasks to be solved, because given the opportunity to receive social services for disadvantaged target groups of the population, thus improving the social exclusion of people at risk the quality of life and contributed to the re-integration into society, as well as reduced the risk of poverty for children and adults.

Target group needs were analysed in the light of its specific needs, such as gender, disability or functional impairment. Implementation of the project is directly focused on health risk reduction, providing services and support to diabetic patients, families caring for children with disabilities, persons with disabilities, thereby reducing the risk of disability. The project implemented by educational and skills workshops contributes to access to employment, mobility, health care, education and lifelong learning.
